秋和Jack分別追隨黑幫老大追哥和皮哥，為爭奪毒品巿場互相廝殺。秋在泰國被皮哥開槍打斷雙腿，Jack也被出賣受傷昏迷，幸獲女友琪所救。兩幫在毒梟擺平下共享香港巿場，暗流卻不住湧動。秋成為身障後意志消沉，女友玲帶他回港找追哥交涉，卻死於槍下，秋也為了報仇殞命。絕跡江湖多時的Jack，得悉噩耗，決心為好友復仇。

雖是雙雄片格局，但杜琪峯打破1980年代香港黑幫類型片的敘事模式，前段寫各事其主的秋和Jack時，採取平行對稱處理，展示二人實力相當，關係亦敵亦友；後段將原本的幫派爭逐，演變成雙雄的個人復仇。充滿異國色彩的泰國外景、顏色濃烈的室內場景，以及劉青雲西部牛仔的粗獷造型等，令電影極具風格。
